knee pain

hi all,

first post for me as i've just started playing cricket with a team.

i had my second session in the nets this past sunday. before i went, i noticed the left side of my knee (i think the LCL) was slightly tender to touch but i thought nothing of it. by the time nets had finished it was painful to walk on.
it's not so bad now, walking is ok but corners and any twisting is sore and so is coming down stairs.

it may be worth explaining my previous history with this knee. when i was 13 i twisted my left knee while kicking a football with my right foot, and fell down in agony. after a visit to the hospital they decided it was a bad sprain and i was put in a splint for 6 weeks, given physio exercises and told no football or other sport for quite some time.
i had problems since, mostly clicking, occassional pain and even had a lump poking out the side of the knee whenever it felt like it. so when i was 20 i had key hole surgery to remove this lump (that still didn't show up on x-ray, as it didn't 7 years before) which i'm told (while barely concious i admit) by the surgeon was a piece of bone about an inch long, and that my knee was now fine and there were no foreseeable problems.

i'm wondering if my previous injury/surgery predisposes me to knee problems like the one i have now, and if theres anything i can do to help prevent them. i normally use tubigrips but i don't know if they're really that effective.

Ligaments have not the blood supply of muscles or tendons so do not recover well at all. If your LCL has been elongated, it remains so, providing little stability and making your knee vulnerable.

This inch long piece of bone concerns me. Are you sure it was 'bone'? If so, what reason were you given for it not appearing on an x-ray?

Were you referred to a physio?

Thanks for your reply liz.

How long would you suggest i rest my knee until beginning some physio? I still have a copy of the strengthening exercises i was given years ago.

I'm not sure it was bone, that's what i was told but didn't think to question the x-ray so unfortunately i don't have an explanation for that. for the size i could only go off the gap he gestured between thumb and index finger.
i had pain just over a year ago and the x-ray came back fine and the pain went away. i asked the nurse at the time if she saw anything there like bone missing and she said no.

I did see a physio but for much longer than i was supposed to. after the surgery my knee didn't stop bleeding for 48 hours, 2 trips to A&E later i was fully stitched up, given antibiotics and put in a splint for 6 weeks.

Rest time depends on the condition of your knee:

Is there any inflammation at the moment?
Is the knee still tender to touch?
Is there any pain or discomfort whilst the knee is at rest?

Are you a right hand pace bowler?

There is very slight inflammation, i can only tell from touch comparing with my right knee.
It no longer hurts to touch and is pain free when resting.

I am a right hand pace bowler, is it that easy to tell? :)

I am afraid so :D.

This makes it hard to get back to cricket any time soon too :(.

If the inflammation increases, ice it... ten minutes within each hour.

You really need to rest until there is no pain with activity. However, this does not mean no movement; you really need to keep the knee mobilised or it will stop working. In the meantime, try to get a good sports massage and stretch your ITB.

It may be worth going to see your GP and see if you can get an x-ray/scan to eliminate anything else.

there's a sports injury place near to me so i'll pay them a visit and get a sports massage and physio

Do keep us informed. I am always keen to hear what sort of service sports clinics provide and if its good, I can pass on the information to others.

i have something similar, when i come off the field at training or after bowling, my knees and ankles hurt a bit, they arent tender, just painful. I'm not quite the dainty kid i once was, but im not obese. I sit just the weight coming down on my legs from running up and bowling, or could it be something else. I bowl right arm pace

i've wondered about the effect of weight too. i've put on about a stone of muscle on in the past 6 months.

i've just been to see the gp who was very confident that its actually my iliotibial band that is inflamed. he advised me to stop running at the gym as he thinks that contributed to my injury, and said crosstrainer or bike is much better.
with my diagnosis changed, is it still worth going to a sports injury clinic?

Yes... I was pretty sure your ITB was contributory too... however, not the cause. You do not need to see a physio but you should get a sports massage. When you get this sorted, and before you bowl again, come back to me and I will talk you through the cause.

Weight can make a difference but it is all relative. What are your weight and height?

This is different; adblunt's issues are biomechanical, yours appear to be tight muscles, particularly calf and hams.

When you say 'hurt', are we talking dull or sharp pain?

Does it feel right inside the joints or at the front or at the back?

How old are you?

What are your weight and height? [You can PM me this info if you would like]:)

hi liz. i've been to the sports injury clinic nearby, which are basically just a physio. he said it's not my ITB because when he was prodding it i felt no pain or discomfort, and thinks it was referred pain from my knee cap which isn't sitting correctly due to a muscle imbalance.. and having a job involving too much sitting.
so i've got some strengthening exercises to do for the time being so i guess i'll see how they go.
the knee certainly feels a lot better, just aching from inside the joint now and then. hoping to get to nets a week on sunday, though he was veery reluctant to say how long i should wait.

Firstly, I am not sure how they can call themselves a sports injury clinic if they do not have a soft tissue specialist. If they are physios, they should practice under 'physios'.

Secondly, you won't feel pain in your ITB from prodding unless it is seriously ruptured :confused:.

I am not saying the cause is your ITB, without seeing you, I could not possibly tell but with your symptoms, it is extremely unlikely that you do not have issues with your ITB.

What exercises were you given?

Keep them up and let us know how the nets go.

It's 1 of 3 premesis they have, i assume like a franchise. Each are slightly different in what they offer. The one i went to was a Physiotherapy and Sports Injury Clinic, but they each seem to be very slightly different. 2 mainly physio and the main original place states they do sports massage and a few other things.
He did say on the phone he's mainly physio but I thought I'd give it a try, get an assessment.
I must say though there is for the past 3 or 4 days a definite ache that feels more inside, so I'm very confused! I'll do the exercises anyway, it can't hurt.
The first exercise is to lay on my back, knees bent then raise my hips as high as i can 20 times. Second is to lay on my side and left the top left into the air 20 times on each side. These are in sets of 50 and are apparently to train my muscles to the movement: Sat down with knees about 90 degrees, lift the front of my foot up using heel as a pivot. Then the opposite of that so pivot with the toes and lift my heels. Lastly, from the same position tilt both knees to the left using the outside of the left foot to pivot and inside of right foot. Then same again but to the right.

Sounds like you are being treated for patellofemoral pain syndrome (PFPS), or runners knee.

Try to get a sports massage, you need to look for a Level 4 (preferably Level 5) therapist. It will cut your recovery time by between approximately one half and two thirds.

Do come back to us after the net session. I am keen to know how this knee copes with your action.

Hi again. I've been doing the physio to strengthen my quads, hopefully it helps but I guess only time will tell.
I went to nets yesterday for a 2 hour session and the knee feels ok. A little bit of pain briefly during it and it feels perhaps a little more.. cracky than before but no pain today.
The coach seemed to like my bowling action although I assume just because it's good for swing doesn't necessarily mean its good for knees!

Usually the case, however, it sounds good. The strengthening will make a huge difference. Keep a watch on it though and if you feel pain, stop. If you would ever like a second opinion, do post a video.
